Dear Resident/Family Member/Responsible Party,

Thank you for entrusting Country Club Rehabilitation Campuses to care for your loved one.

First and foremost, we want everyone to understand that the number one focus of our Country Club Campuses is to ensure that our residents and employees are safe and continue to receive the same quality care in a healthy environment!

Country Club Rehabilitation Campuses are still taking the spread of COVID-19 very seriously and we are taking our guidance from the Centers for Disease Control and Prevention (CDC) and our local health departments to ensure we are adapting our precautions to prevent the spread of this virus.

Our campuses continue to adapt its policies and procedures for each of our facilities. Some of these policies include:

Visitation: Per Governor Mike DeWine’s order, effective July 20, we are opening up our visitation policy for both our Assisted Living/Apartment residents and our Nursing Home Residents. The following guidelines will be adhered to:

• Temperatures are taken upon entrance to any facility

• All employees/visitors are completing proper hand washing with soap and water for at least 20 seconds

• All appointments should be booked AT LEAST three calendar days in advance, and will be granted based on availability. You must contact the facility Activity Director to schedule an appointment.

• Visitors and residents will be screened for signs/symptoms of COVID-19. Any symptoms will be reviewed by CCRC nursing personnel to determine if the visit is safe for our residents and staff.

• Finally, all visitors and residents will be wearing face masks at all times and will be required to maintain social distancing guidelines.

Dining: Country Club Rehabilitation Campuses are maintaining communal dining and activities while adhering to social distancing guidelines where possible.

Other Responsive Measures Country Club Campuses Have Taken: Below is a list of other measures Country Club Rehabilitation Campuses have taken in order to keep our residents and employees safe and free of COVID-19. This list is all inclusive but not limited to.

• Offering the COVID-19 vaccine to all staff members and employees. At all of our CCRCs, we encourage everyone to get the vaccine now that it is available to all people.

• Constant communication with our residents and families about the current COVID-19 precautions and procedures taking place at each facility.

• Connecting residents with their loved ones through Skype and usage of iPads for Face Timing

• Notification to all suppliers of our new visitor policies and the usage of only necessary supply distributers including food, medications and medical equipment

• Signage posted at all entrances and throughout facilities regarding Infection Control policies and procedures

• Local community Facebook page updates 

• Signage posted at all entrances and throughout facilities regarding Infection Control policies and procedures

• Increased cleaning and environmental sanitation protocols for COVID-19 including: the installation of the iWave system, and usage of industrial foggers, temperature monitoring kiosks, and microSURE
